Before you start. It is a criminal offence to give a Notice to an authorised celebrant, or to sign it,
knowing it contains a false statement or an error (section 104, Marriage Act 1961).
Your completed, signed Notice must reach your celebrant at least one month, and not more than
18 months, before your ceremony. If you cannot obtain a piece of information after reasonable
enquiry, write unknown in that box — a statutory declaration explaining why is then needed for
everything except items 11–16 (the parents' details).

Person 1 — your details
Items 1 to 9 of the Notice. Use your full legal names exactly as they appear on your birth certificate or passport — not a shortened or married name.

Items 11 to 16. Give each parent's name as it is now, and the name they were given at birth. If you genuinely cannot find out a detail, write unknown — unlike the rest of the form, these items need no statutory declaration.
